CSHS will be hosting physicals on Wed, April 24th during 7th period athletics and after school. We ask that students sign up for a time slot to help space out the number of participants at one time. Cost is $20 cash, check (made to CSISD) or online in advance with Intouch (preferred). Parents can find information as well as the LINKS TO SIGN UP for their time slot at cshscougarclub.com/physicals Please go to cshscougarclub.com/physicals to download and print the form. Complete the left column and both student and parent signatures at the bottom. Bring this to the event with you. Parents do not need to attend.  Link
